The Chronic Disease Management RN renders care to patients in the clinic setting working as a valuable member of the health care team making independent and dependent decisions under the direction of a provider and in collaboration with a core team. In this role, you will coordinate a weekly review of caseload to develop plans that provide optimal care to the patient.
The Chronic Disease Management RN is a co-leader in the core team, and is responsible for collaborating with services, both internal and external to the clinic to facilitate care transition to effectively meet the patient s needs by providing health education with a focus on self-management, prevention and wellness, based on the patient s goals. This position provides care to patients and families, supports and implements patient safety and other safety practices as appropriate.
- Schedule
- Full-time | 68 hours every two weeks | Melrose Clinic
- 4 shifts per week | 8:00 am – 5:00 pm
- No weekends or holidays
- Pay and Benefits
- Pay based on experience and degree
- Associate/Diploma in nursing starting at $37.27 per hour
- Bachelor in nursing starting at $38.56 per hour
- Masters in nursing starting at $39.64 per hour
- Pay range: $37.27 - $62.18 per hour

- Qualifications
- Graduate of an accredited school of nursing
- Registered Nurse licensure in the state of Minnesota
- Basic Life Support (BLS) American Heart Association (AHA) Healthcare Provider card
- Minimum of 2 years of successful clinical experience, preferably in chronic disease education
- Certified Diabetes Educator National Certification required within 2 years of employment
