- Complete routine and PRN hospice visits throughout your assigned territory.
- Perform comprehensive nursing assessments and ongoing symptom management.
- Provide pain management, wound care, medication education, and comfort-focused interventions.
- Partner with physicians, social workers, chaplains, hospice aides, and families to coordinate individualized plans of care.
- Maintain accurate, timely documentation within the EMR.
- Active Minnesota Registered Nurse (RN) license.
- Hospice experience preferred, with consideration given to nurses from home health, ICU, oncology, palliative care, or other end-of-life care settings
. - Excellent assessment, critical thinking, and communication skills.
- Confidence working independently in a field-based environment.
- Reliable transportation and a valid driver's license.
