Full-Time | Monday–Friday, 8:00 AM – 5:00 PM | On-Call Rotation

Compensation: $95,000 – $130,000 annually plus incentive visit opportunities

As a Floating RN Case Manager , you'll provide experienced clinical coverage throughout our service area, stepping into territories where additional Case Management support is needed most.

Your role helps us make sure one simple thing remains true:

When a patient needs a nurse, we have a nurse ready to care for them.

You're the Nurse Who Keeps Care Moving

This isn't a backup position.

And it isn't about filling holes in a schedule one visit at a time.

You'll step into a territory, take ownership of an assigned caseload, and help provide the consistency patients and clinical teams need.

Assignments will typically last approximately three months , allowing you to establish yourself within an area rather than bouncing between territories every few days.

You’ll learn the patients.

Understand their plans of care.

Work alongside the local team.

Coordinate with physicians.

Follow their progress.

And help make sure their care doesn't lose momentum.

When the need shifts somewhere else,
you'll be ready to do it again.

How Floating Works

Your assignments will rotate throughout our 21-county Greater Twin Cities service area based on patient census, staffing, and clinical needs.

Rather than constantly changing locations, you'll generally support a territory for approximately three months at a time .

During that assignment, you'll:

  • Manage an assigned home health caseload
  • Become familiar with the patients and caregivers in your territory
  • Coordinate care throughout each patient's episode
  • Work alongside the clinical team supporting that area
  • Help absorb census growth or temporary coverage needs
  • Provide consistency while the territory's longer-term needs are addressed

Then you'll transition to the next area where your experience can make the greatest impact.

You're not randomly floating. You're being strategically deployed. What You'll Do

As an RN Case Manager, you'll be responsible for the complete clinical picture of your assigned patients.

You'll:

  • Manage an assigned home health patient caseload
  • Complete OASIS Start of Care, Recertification, and Discharge assessments
  • Develop and update individualized plans of care
  • Perform wound care, medication management, and other skilled nursing services
  • Educate patients and caregivers on diagnoses, medications, and treatment plans
  • Monitor patients for changes in condition
  • Communicate clinical concerns directly with physicians
  • Coordinate with PT, OT, SLP, social work, and other interdisciplinary clinicians
  • Help prevent avoidable emergency department visits and rehospitalizations
  • Maintain timely and accurate documentation using Homecare Homebase (HCHB)
