About the Role Baywood Home Care is seeking a compassionate Registered Nurse (RN) Case Manager to coordinate care, support patients and families, and help individuals maintain their independence in the comfort of their own homes. • Competitive pay — $37.44–$45.00/hour, based on experience
• Flexible scheduling — create a part-time schedule up to 20 hours/week, with hours coordinated around both your availability and patient needs

• Optional paid on-call opportunities — on-call participation isn't required, but additional compensation is available for nurses who choose to take on-call shifts

• Conduct patient assessments and develop individualized care plans
• Coordinate services with physicians, therapists, caregivers, and community resources
• Monitor patient progress and adjust care plans as needed
• Educate patients and families on medications, treatment plans, and wellness strategies
• Maintain accurate clinical documentation and regulatory compliance

• Current Minnesota Registered Nurse (RN) license required
• Home health, case management, or transitional care experience required
• Strong assessment, communication, and care coordination skills
• At least 21 years old
