We are seeking a  
** Licensed Practical Nurse (LPN)
** to join a home health team serving patients throughout the  
** Chisago City, Minnesota area** . This full-time opportunity allows you to provide individualized nursing care directly in patients' homes while working alongside a supportive and experienced clinical team.

This position is an excellent fit for an LPN who is organized, compassionate, comfortable working independently, and interested in developing meaningful relationships with patients and their families.

** Monday-Friday, 8:00 AM-5:00 PM*
* +  **$66,000-$74,000 annually*
* + Compensation determined by nursing experience and a point-based system

+ Participation in an established  
** on-call rotation is required*
* + Local travel to patient homes within the assigned territory

** What You'll Be Doing*
* In this role, you will help patients safely manage their healthcare needs while remaining in the comfort of their own homes. Responsibilities may include:

+ Providing skilled nursing visits according to physician orders and individualized plans of care

+ Assessing patients and monitoring changes in their condition

+ Administering prescribed medications, treatments, and nursing interventions

+ Educating patients and caregivers regarding medications, diagnoses, treatments, and ongoing care needs

+ Completing accurate and timely clinical documentation

+ Communicating changes in patient status to the appropriate members of the care team

+ Collaborating with physicians, therapists, case managers, and other healthcare professionals

+ Supporting patient safety, comfort, independence, and overall quality of life

+ Participating in the required on-call rotation and responding to patient needs as appropriate

** Qualifications*
* We are looking for an LPN who can combine strong clinical skills with excellent communication and time management.

+ Graduate of an accredited  
** Practical Nursing or Vocational Nursing program*
* + Active and unrestricted  
** Minnesota LPN license*
* + Current  
** CPR certification*
* +  
** One or more years of clinical nursing experience preferred*
* + Previous  
** home health experience is preferred but not required*
* + Valid driver's license and dependable transportation

+ Ability to travel between patient residences within the assigned service area

+ Strong clinical documentation, communication, and organizational skills

+ Ability to independently organize patient visits and manage changing priorities
