Many are navigating challenges such as homelessness, disconnection and complex issues long before they reach adulthood. This role exists to meet young people where they are and offer support, consistency and safety. It is for someone who shows up and walks beside them as they move toward independence.

About the role

This role provides proactive and strengths-based support to young people who are marginalised, disconnected from services or experiencing or at risk of homelessness. You will deliver consistent assertive outreach, case management, advocacy and immediate practical assistance that improves access to housing, health, education, employment and community supports.

The position covers both Darwin and Palmerston, and you will work across these communities as part of your outreach and case management responsibilities.

This is a full-time permanent role within Youth Services and reports to the NT State Manager. Pay is in accordance with SCHADS Level 4.

How you will make an impact

  • Undertake assertive outreach to respond to the immediate needs of young people
  • Build culturally appropriate and strengths focused relationships that promote trust and self determination
  • Provide person centred support aligned with TSA's Youth Services Model of Care
  • Develop, implement and review case plans that reflect the goals and needs of young people
  • Conduct ongoing assessment of safety, risk and wellbeing from first contact through to exit
  • Support young people to access housing, health, education, employment and community services
  • Advocate for young people and accompany them to appointments with specialist providers
  • Use creative and activity-based outreach methods to engage young people and raise awareness of youth issues
  • Maintain accurate case notes, records and reporting in line with TSA requirements
  • Identify and escalate risks to ensure the safety of clients, staff and the community
  • Contribute to continuous improvement, service development and professional learning

What you will bring

  • Degree qualification in Youth Work, Community Development, Social Work or a related human services or education field
  • Demonstrated experience supporting young people with complex needs
  • Strong understanding and experience in trauma informed and culturally responsive practice
  • Ability to build trust quickly and work effectively with young people in non-traditional settings
  • Knowledge of youth homelessness, education, youth justice and community service systems
  • High level communication, relationship building and advocacy skills
  • Ability to work independently, safely and with sound professional judgement
  • Commitment to TSA values and inclusive, person-centred practice
  • Current NT Working with Children's Check (Ochre Card)
  • Valid NT Drivers Licence
  • Valid First Aid Certificate
