Program Coordinator (Site Supervisor)
Opportunity Matters is seeking a Site Supervisor for a house in Sartell, MN. At this home lives a high-energy, fun-to-work-with individual who likes basketball, walking to the park, playing video games, and going places. This site needs someone who can match that energy, be with the individual at all times, step in during interfering behaviors, and support appropriate social interactions. The person served requires active supervision at all times, especially during encounters with people in the community and/or visitors to the home.
The ideal candidate can recognize antecedents, know when the person needs downtime and space, and support the individual through behavior cycles. The Site Supervisor will train and oversee staff so that the team understands how to work with the individual consistently in following the programs and service plan. Our Site Supervisors are assigned to one home site, but they understand how the Organization works and help out at other sites as needed.
The schedule is mainly 8a-4p M-F. May be required to help cover other times and may work occasional weekend shifts if needed.

The Program Coordinator oversees one program under the direction of the Program Director. The Program Coordinator ensures compliance with all aspects of the program related to the persons served, the Organization, the employees, and property. The Program Coordinator is responsible for providing effective leadership and accountability to foster growth and development in the team of Direct Support staff. The Program Coordinator provides comprehensive personal care, guidance, and appropriate support tailored to the needs of each person.

- Maintain knowledge of the Direct Support Professional job description, perform, train, and oversee all of the duties of that position.
- Foster a positive living environment for persons served and a positive working environment for staff.
- Build and maintain rapport with persons served, their support teams, staff, co-workers and Org leadership.
- Act as an appropriate role model for persons served and staff.
- Actively supervise staff to ensure programs are implemented and the tasks at the site are being completed.
- Work with Program Director to develop, improve, and implement program planning for persons served.
- Manage the weekly grocery shopping, plan menus, write grocery lists, adhere to budget. Ensure personal needs shopping is done.
- Maintain a clean and sanitary site/program.
- Communicate with Supervisor on persons served care plans and overall operations of the site/program.
- Maintain records, documents, charts, and client information, legibly. Ensure staff are recording information in an accurate and timely manner.
- Track money of persons served, oversee counting procedures, and secure the money at the site.
- Immediately respond and address emergency situations.
- Communicate with and assist other staff positively and professionally. Deliver disciplinary action if needed. Provide on-the-spot feedback.
- Assist with and complete duties that need to be done at the site/program, following Organization policies and procedures.
- Order medications and maintain Medication Administration Records.
- Ensure there is adequate shift coverage at all times. This may include, but is not limited to, staying beyond a scheduled shift or covering/splitting open shifts with DSPFT and/or other Direct Support Staff.
