Under direction, performs work in providing social work services on cases involving complex social problems and social treatment plans; performs responsible and difficult casework, directs service programs, or provides work direction to paraprofessional staff; and performs related work as assigned.

Responsible for case management/ care coordination of person receiving Alternative Care, Elderly Wavier, Nursing Home and/ or MSHO/ MSC Services

  • Makes face to face visits to each client receiving alternative care, elderly waiver and/or MSHO/MSC+ services at least every six months or as health plan requires.
  • Maintains files on each client receiving alternative care, elderly waiver, community well and nursing home care coordination/case management. The file in documents will be maintained in Appxtender, documentation will be maintained in SSIS.
  • Completes an annual assessment, support plan, and enter a service agreement for individuals receiving waivered services. Completes an annual HRA and support plan for individuals receiving community well care coordination. Completes a nursing home annual assessment for individuals receiving nursing home care coordination.
  • Develops community support plans according to the DHS Person Centered Protocol.
  • Carries out case management per DHS and health plan requirement for clients enrolled in a health plan.
  • Responsible to be knowledgeable, stay current, and abide by the requirements set forth in the alternative care/waiver program.
  • Responsible to do early intervention and relocation service coordination activities.
  • Responds to individuals and family members requesting assistance and assessing their need for community-based home care and helping them to access services which will help the individual to avoid institutionalization.
  • Provides assistance to patients and nursing home staff who are planning nursing home discharge. Identify patients’ potential needs upon re-entry into the community and assist them in accessing appropriate services.
  • Provides information and education to the general public regarding availability of services authorized under Home and Community Based Services.
  • Collaborates with the Care Coordination Team.
  • Provides support to the other Care Coordinators in navigating the healthcare and waiver programs.
  • Obtain guidance and direction from the Care Coordination team members when guidance is needed to implement the healthcare and waiver programs according to DHS statute and health program guidelines.
Intake Crisis Intervention
  • Facilitates early treatment and rehabilitation and education for various targeted populations and the general public.
  • Provides information and referral to persons contacting our agency.
Conduct MN Choice Assessments as assigned
  • Contacts client assesses need for MN Choices assessment for any initial assessments assigned.
  • Completes MN Choices assessments within required timeline.
  • Provides certified assessors with the information to completed reassessment 60 days prior to the end of the SA agreement and according to the unit protocol.
  • After initial MN Choices assessment complete CSP/CSSP within required paperwork related to the assessment.
Additional Duties and Responsibilities
  • Attend training/workshops outside agency as applicable/available.
  • Complete DHS web-based training when appropriate.
  • Participate in annual individual evaluations.
  • Answers co-worker questions: consults with co-workers when needed.
  • Reads routed material in a timely manner.
  • Researches manual materials; on-line materials.
