We are seeking a full-time Case Manager to support our Emergency Department team, focusing on discharge planning, patient resource connection, and promoting positive outcomes
.
- Coordinating care from pre-admission to post-discharge,
- Facilitating interdisciplinary communication and early intervention,
- Evaluating outcomes using clinical, financial, and utilization data,
- Advocating for patients to resolve issues within healthcare delivery,
- Connecting patients with community resources, and
- Supporting families through crisis intervention and education.

- Bachelor’s Degree in nursing or Social Work,
- Unencumbered RN license (for Care Managers) or licensed Social Worker credential,
- Basic Life Support (BLS) certification within six weeks (for Social Workers),
- Strong communication, critical thinking, and problem-solving skills,
- Ability to work in diverse healthcare settings and collaborate with multidisciplinary teams.
