Healthcare Care Manager or Social Worker

We are seeking a full-time Case Manager to support our Emergency Department team, focusing on discharge planning, connecting patients with community resources, and ensuring positive outcomes.

Key responsibilities include coordinating care from pre-admission to post-discharge, facilitating interdisciplinary communication, evaluating outcomes through clinical and utilization data, and promoting early intervention.

Social Workers provide crisis intervention, assist families in understanding medical situations, coordinate healthcare programs, respond to abuse or neglect, develop care plans, and connect patients with appropriate services. They also advocate for patients and participate in care conferences and transitional rounding.

Qualifications include a Bachelor’s Degree in Nursing or Social Work, an unencumbered RN license in Minnesota (for Care Managers), and Basic Life Support (BLS) certification (for Social Workers).

Skills required are critical thinking, problem-solving, effective communication, independence in judgment, and collaboration with healthcare teams and community agencies.
