Job Overview
Terralume Signature Memory Care is a new, boutique community designed exclusively for individuals living with Alzheimer's and other forms of dementia. Located off Wayzata Boulevard in Minnetonka, we are opening Spring 2026 and building a team of compassionate, adaptable professionals who want to help shape something truly special.
The Registered Nurse (RN) is responsible for ensuring the safety and health needs of the residents are met.
This Nursing schedule includes:
- 8 hours every two weeks
- Day Shift
- Every other Saturday
- Maintain current assessment and documentation of resident health status:
- Perform initial and ongoing assessments per the applicable Minnesota Assisted Living regulations and Nurse Practice Act. Ensure assessments are completed every 90 days per regulatory requirements.
- Work with the resident and/or responsible party through collaboration, education, and timely communication to ensure appropriate service and placement.
- Ensure the resident has an up-to-date service agreement at the time of admission, with any changes in services, and annually.
- Develop, maintain, and implement individual emergency plans based on health status and Advance Care Planning documents, including POLST.
- Maintain clinical charts and progress notes using a computerized documentation system.
- Communicate changes in resident conditions to medical and other relevant individuals and perform the appropriate follow‑up to health needs.
- Coordinate resident health services:
- Facilitate coordination of services with internal and external providers.
- Assure that all physician orders are signed and implemented per the applicable Minnesota Assisted Living regulations.
- Work with resident and/or representative(s) to assure delivery of appropriate services and funding.
- Manage and oversee medication and treatment administration. Assure medications and supplies are ordered in a timely manner.
- Assure medication and treatment practitioner orders are renewed at least annually and as necessary when there is a change in medication and/or treatment. Minnesota Assisted Living regulations will be followed in obtaining and implementing practitioner orders.
- Assure medication boxes, bubble packs, and/or syringes are filled per medication orders/changes for medication administration by designated staff.
- Assure that all medication administration procedures are correctly followed. Take appropriate action with staff regarding any problems or issues.
- Assure accuracy of medication and treatment records, including documentation of administration and appropriate signatures.
- Monitor work and performance of licensed and unlicensed staff:
- Meet with staff to coordinate care and address care issues.
- Plan and assign/delegate work to staff within established guidelines.
- Interview, train, and orient new employees as directed.
- Assist and support staff to complete assignments as needed.
- Assist the Director of Health Services (DHS) in monitoring and evaluating staff performance.
- Schedule and complete Supervisory Visits.
- Comply with Federal, State, and Local Laws.
- Assist DHS in scheduling activities that support efficient and effective utilization of staff as needed.
- Perform any other duties as delegated by the Director of Health Services.
- Support DHS to act as RN on call during scheduled times:
- Answer calls and gather information related to assisted living community residents' health and safety.
- Take action as appropriate to the situation.
- Work collaboratively with identified internal and external support providers.
- Actions taken may require phone directives and/or traveling to the assisted living community to resolve the situation.
- Report to the DHS and/or Executive Director if further follow‑up is needed.
- Associate’s Degree in Nursing
- Less than 1 year nursing experience
- MN Registered Nurse (RN) License
- Basic Life Support (American Heart Assoc or Red Cross)
- Bachelor of Science Nursing
- 1 year nursing experience
- Prior supervisory experience
- Prior experience in home care or assisted living environment
