The Hospice RN Visit Nurse is responsible to plan, organize and direct home care services and is experienced in nursing, with emphasis on community health education/experience. The professional nurse builds from the resources of the community to plan and direct services to meet the needs of individuals and families within their homes and communities.
Position:
RN Visit Nurse for Hospice
Salary Wage Range: $85,000-$91,300 annually, depending on experience
Service Area:
Southwest Metro (coverage area: Waconia, Carver, Chaska, Jordan, Chanhassen, Savage, Minnetonka & surrounding).
Plus, mileage reimbursement of 72.5 cents per mile.
Schedule:
Full-Time 1.0 FTE, Monday – Friday 8:00am-5:00pm, (weekend rotation: 5 shifts per quarter / holidays: 2 per year / no on-call)

- Completes an initial assessment of client and family to determine home care needs. Provides a complete physical assessment and history of current and previous illness(es).
- Initiates the plan of care and makes necessary revisions as client status and needs change.
- Uses health assessment data to determine nursing diagnosis.
- Develops a care plan, which establishes goals based on nursing diagnosis and incorporates therapeutic, preventive, and palliative nursing actions. Includes the client and the family in the planning process.
- Initiates appropriate preventive and rehabilitative nursing procedures. Administers medications and treatments as prescribed by the physician. Provides direct client care as defined in the State Nurse Practice Act.
- Counsels the client and family in meeting nursing and related needs.
- Provides health care instructions to the client as appropriate per assessment and plan of care.
- Identifies discharge planning needs as part of the care plan development and implements prior to discharge of the client.
- Acts as Case Manager when assigned by Clinical Supervisor and assumes responsibility to coordinate patient care for assigned caseload.
- Lead by example in establishing rapport with clients, clinicians, partners, and the community.
- Ensure positive/professional communication within the team
- Customer service resolution; escalates concerns
- Prepares clinical notes and updates the primary physician when necessary and at least every 60 days.
- Communicates with the physician regarding the client’s needs and reports any changes in the client’s condition; obtains/receives physician’s orders as required.
- Communicates with community health related persons to coordinate the care plan.
- Timely completion of all documentation as it relates to the care plan by 10am the next day
- Understands and complies with all the rules and regulations that govern the responsibilities of a registered nurse
- Completes orientation and annual training requirements as assigned
- Participates in Lifespark’s Event Management process including initiating reports, escalation of events and investigations
- Promotes safety in the workplace through real time coaching and participation in safety awareness training
- Manages client supplies ensuring appropriate DME provider is utilized.
- Schedules and keeps member appointments
- Organizes travel efficiently and documents mileage accurately within EMR
- Other duties as assigned
- One to two years of recent acute care experience in an institutional setting. One year of home health care experience preferred.
- Experience in customer service/direct member contact field preferred
- Experience supervising caregivers
- Graduate of an accredited school of nursing
- Must pass a MVR check based on Lifespark company policy
