Overview
Home Care Licensed Practical Nurse (LPN):
Enjoy independence, autonomy, and flexibility in the field to visit patients one on one in their home environment. The LPN will provide care coordination, disease and medication management, skilled wound care, catheter management, ostomy education, infection prevention, and home safety.
- Pay Range: $30.42 - $36.98/hour

Full-time | Days, Monday
- Friday. There will be a weekend and holiday rotation. Work approximately one weekend every 4-6 weeks and 2 holidays per year. The weekend and holiday rotation is dependent upon staffing levels.
This position will work in the field helping patients in their home setting. Our office is in Roseville, MN and the general territory for this position would be the southwest metro.
Qualifications- Current license as an Licensed Practical Nurse (LPN) in the State of MN. License is active and in good standing.
- 2 years experience as an LPN is preferred.
- Able to travel within geographic area serviced by the office. Valid driver's license, reliable transportation, and proof of auto insurance are required.
- CPR Certification
- Participate in ongoing interdisciplinary assessment of the patient.
- Provide services that are ordered by the physician as indicated in the plan of care.
- Recognize and report changes in the patient’s condition to the RN.
- Participate in the development and evaluation of the plan of care in partnership with the patient, representative (if any), and caregiver(s).
- Counsel and educate the patient and family in meeting healthcare-related needs.
- Complete timely and accurate clinical notes including addressing patient’s progress.
- Get the opportunity to see patients in their home environment to get a true representation of each patient’s needs.
