This is more than completing an admission. It's about helping families understand their options, building trust from the very first visit, and ensuring every patient begins hospice care feeling supported, informed, and cared for.

• Travel to patients' homes, assisted living communities, hospitals, and skilled nursing facilities to complete hospice admissions.
• Perform comprehensive clinical eligibility assessments and initiate hospice services.
• Lead conversations about goals of care, hospice philosophy, Medicare benefits, and what families can expect .
• Coordinate seamless transitions into hospice with physicians, hospital discharge planners, social workers, intake coordinators, and the interdisciplinary team .
• Build trust with patients, caregivers, and referral partners from the very first interaction.
• Complete accurate, timely EMR documentation while ensuring regulatory compliance.

• Active Minnesota Registered Nurse (RN) license.
• Experience in hospice, oncology, ICU, palliative care, home health, emergency medicine, or another complex clinical setting.
• Outstanding communication, assessment, and relationship-building skills .
• Confidence leading sensitive conversations with patients and families.
• Ability to work independently in field-based role .
• Valid driver's license and reliable transportation.
