- Travel to patients' homes, assisted living communities, hospitals, and skilled nursing facilities to complete hospice admissions.
- Perform comprehensive clinical eligibility assessments and initiate hospice services.
- Lead conversations about goals of care, hospice philosophy, Medicare benefits, and what families can expect.
- Coordinate seamless transitions into hospice with physicians, hospital discharge planners, social workers, intake coordinators, and the interdisciplinary team.
- Build trust with patients, caregivers, and referral partners from the very first interaction.
- Complete accurate, timely EMR documentation while ensuring regulatory compliance.
- Active Minnesota Registered Nurse (RN) license.
- Experience in hospice, oncology, ICU, palliative care, home health, emergency medicine, or another complex clinical setting.
- Outstanding communication, assessment, and relationship-building skills.
- Confidence leading sensitive conversations with patients and families.
- Ability to work independently in a field-based role.
- Valid driver's license and reliable transportation.
