The Diabetes Care – Education Specialist provides comprehensive diabetes and prediabetes education to support patients in developing the knowledge, skills, and behaviors needed for effective self-management. This role delivers evidence-based education and ongoing support tailored to each patient’s individual needs, goals, and life experiences. Working collaboratively with an interdisciplinary care team, the Specialist helps improve clinical outcomes, enhance quality of life, and promote sustained self-care behaviors.
Through clear communication, patient-centered teaching, and consistent follow-up, this position plays an important role in empowering patients to actively manage their health.
Key Responsibilities:
Provide individualized and group diabetes and prediabetes education using evidence-based standards to support informed decision-making and self-management.
Assess patient learning needs, readiness, and barriers to develop personalized education plans that support long-term behavior change.
Educate patients on nutrition, medication management, glucose monitoring, insulin therapy, and use of diabetes-related technology as appropriate.
Collaborate with physicians, nurses, dietitians, and other care team members to coordinate care and reinforce consistent treatment plans.
Document education encounters, patient progress, and outcomes accurately within the electronic health record in accordance with organizational standards.
Participate in quality improvement initiatives and maintain professional competency through continuing education and certification requirements.
Licenses and Certifications
Required:
Current or temporary RN license in ND (or compact RN licensure) required.
Diabetes Care and Education Specialist certification required upon hire.
Certification in insulin pump training required upon hire.
Educational Requirements:
RN program as evidenced by licensure.
Experience Requirements:
Experience acute patient care and chronic disease management is preferred.
Special Skills or Training Requirements:15 hours of continuing education annually or maintenance of Diabetes Care and Education Specialist certification from a nationally accrediting body.
Physical Requirements:
This role requires frequent sitting, standing, walking, and use of computers and educational materials throughout clinic and education settings. Occasional bending, reaching, and light lifting of teaching supplies or equipment may be required. Adequate vision, hearing, and communication abilities are essential for patient education, assessment, and effective interaction with individuals and groups.
Environmental Requirements:
The Diabetes Care – Education Specialist works primarily in outpatient clinic, education, and healthcare office environments. The role involves regular interaction with patients who may have chronic conditions and occasional exposure to illness. Adherence to infection prevention practices, safety protocols, and confidentiality standards is required while delivering patient-centered education and support.
