What you will do:

Provides the exercise prescriptions and consultations, coordinates exercise functions for the program. Conducts the wellness classes and presentations. Reports to department manager.

Key Responsibilities:

* Uphold the organization's mission, core values, and compliance with all health, safety, and medical regulations.

* Review patient evaluations and exercise histories to develop and update individualized exercise prescriptions in collaboration with healthcare providers.

* Monitor patient health and safety during sessions, document progress, and adjust plans as needed.

* Educate patients and families on disease processes, home exercise programs, stress management, and behavior modification.

* Lead group exercise, relaxation, and educational sessions; assess patient responses and readiness for change.

* Participate in discharge planning, team meetings, process improvement, and ongoing program development.

* Maintain required competencies, certifications, and organizational standards.

What you will need:

* Bachelor's degree in physical therapy, Allied Health or Exercise Science. Certified by ACSM as Exercise Specialist or master's degree in Exercise/Exercise Physiology.

* Current license to practice in the state of Indiana (if physical therapist). Certified in Basic Life Support. ACLS Certification preferred.

* Prefer two years previous experience in adult fitness and/or cardiac rehabilitation programs.
