Physical Therapist – PRN
Location:
Mishawaka, IN; 4455 Edison Lakes Pkwy, Suite 100, Mishawaka, IN 46545, USA.
Job Type: PRN
DescriptionSouth Bend Orthopedics is seeking a PRN Physical Therapist for our Unity Hospital location. Unity is an elective orthopaedic surgical hospital.
Responsibilities and Duties- Assess patient condition, establish goals, treatments and care plan in conjunction with patient
- Ensure appropriate treatment is performed in interdisciplinary fashion
- Document findings, treatments, and other patient information for medical record
- Educate patient on appropriate care and use of various therapies/exercises
- Comply with appropriate clinic policies
- Serve as information/education resource for patients, families, and staff.
- Other duties as assigned.
- BS, MA, MS, or DPT in Physical Therapy
- Must be licensed in the state of Indiana.
- Outpatient
- Physical Therapy License
- CPR Certification
- In person
