RN Indiana license or CMA certification required.
- Actively demonstrates the organization’s mission and core values and conducts oneself at all times in a manner consistent with these values.
- Knows and adheres to all laws and regulations pertaining to patient health, safety and medical information.
- Provides wellness education programs on a regular and on-going basis. Provides screenings and education based on the test results.
- Instructor of health and wellness programs.
- Participates in program training as prerequisite for each program and demonstrates competency with program material and format.
- Schedules program classes with manager/coordinator.
- Keeps attendance list of participants and provides outcome information to designated personnel.
- Assists in developing new education programs based on community needs.
- Utilizes effective group leadership skills.
- Maintains contact with appropriate site staff. Provides outcomes to coordinator or manager.
- Models appropriate behavior for class participants by a healthy, positive, and upbeat attitude.
- Seeks answers to participant’s questions and reports back to them.
- Participates in health fairs, educational presentations and other Community Health and Well Being (CHWB) activities per the request of the director of CHWB.
- Attends department staff meetings and participates in Process Improvement, if applicable.
- Utilizes computer programs as appropriate for assigned tasks.
- Performs other duties consistent with purpose of job as directed.
