Benefits- Medical Insurance – Two health insurance plans (HSA or PPO co-pay) are available to choose from for staff with weekly scheduled hours of 32 or more as well as power weekend employees. Coverage is effective the first day of the month following employment.
- Dental & Vision – Dental and vision insurance are available to staff with weekly scheduled hours of 32 or more and power weekend employees. Coverage is effective the first day of the month following employment.
- Wellness Benefit – Many routine testing, exams, and immunizations are covered 100%.
- Health Savings Account – CHC contributes up to $500 per year at the beginning of the year.
- Flexible Spending Account – Able to deduct up to $2,000 (pre‑tax) each calendar year for reimbursement of uninsured medical, dental, or vision expenses.
- Employee Assistance Program (EAP) – CHC provides an EAP to help non‑PRN employees, their spouses, and eligible children in coping with personal problems and stress. CHC will pay the cost of four counseling sessions per employee and eligible family members each calendar year. This is effective immediately upon employment. Employees may contact the EAP provider to directly schedule confidential appointments. The number of employees participating is the only information shared with CHC.
- Healthy Lifestyle Benefit – Employees who participate in CHC’s medical insurance are eligible for reimbursement of 50% of the cost up to $300 per calendar year for participating in the following: gym membership (single membership rates), weight loss counseling or education, organized exercise programs such as Zumba, Yoga or Pilates.
- 403B / Roth 403B – Employees regularly scheduled to work 20 hours or more per week are eligible to participate in CHC’s retirement plan following completion of their 90‑day probationary period. Vesting is 100% upon enrollment. CHC will match at 25% the first $16,000 of the employee’s contribution annually.
- Accidental Death and Dismemberment Insurance – CHC provides one times the annual earning in AD&D insurance for staff with weekly scheduled hours of 32 or more as well as power weekend employees. This is effective the first day of employment.
- Life Insurance – Effective the first day of employment, CHC automatically provides life insurance equal to one times the employee’s annual salary or a minimum of $20,000 for power weekend employees and all staff with weekly scheduled hours of 32 or more.
- Employee Peer Recognition program
- Annual anniversary/retention bonus – Upon completion of each year of employment, CHC will give $100 per year with a maximum of $1,000.
- Competitive Time Off – CHC’s paid time off (PTO) policy provides employees with flexible earned time off for vacation, illness, personal matters and other reasons. Staff with weekly scheduled hours of 32 hours or more are eligible to accrue PTO.
Responsible for telephone assessment, problem solving, and direction of the interdisciplinary team as needed to address patient/family needs related to the hospice terminal diagnosis. Accountable and reports to the Patient Care Coordinator.
- Assesses patient/family needs, evaluating symptoms and interventions via telephone.
- Executes a therapeutic regime of treatments and medications as prescribed by the attending physician or Agency Medical Director, using nursing judgment.
- Utilizes nursing judgment to provide symptom management related to the patient's diagnosis through family teaching.
- Initiates or facilitates referral of patient and/or family to appropriate Agency or community resources.
- Collaborates with the interdisciplinary team, patient, and family to provide a patient/family focused plan of care.
- Provides supervision of LPN and CNAs in accordance with Agency policy and regulatory requirements.
