• Full-time Hospice LPN Position
• Schedule:
  
  Monday thru Friday, 8a-5p
• Available territory openings:
  Mishawaka and surrounding communities

• Performs physical and psychosocial assessment on all hospice patients.
• Ability to adequately assess and reassess pain. Utilizes appropriate pain management techniques. Educates the patient and family regarding pain management.
• Responds to the physical, mental, social and emotional needs of terminally ill patients and their families.
• Assesses patient’s condition and coordinates with RN Case Manager.
• Participates in case conferences to discuss multidisciplinary team responsibilities, pain management, bereavement counseling, etc.

• Experience:
  
  One year Hospice experience preferred, but not necessary
• 1 year of LPN experience required
• Credentials:
  Licensed LPN in State of Indiana
• Current driver's license
• Must have reliable transportation and insurance
