About the Position
We are seeking a
compassionate and experienced Nurse Practitioner to join our team in Mishawaka,
Indiana. This full-time role works closely with clinical staff and reports to
the Site Medical Director and Site Operations Director, providing high-quality,
patient-centered outpatient care within a Patient-Centered Medical Home (PCMH)
model.

Responsibilities:

Provide comprehensive
outpatient primary care and manage patient panels to ensure continuity of care
Develop and implement
effective patient care plans, including referrals as appropriate
Perform periodic health
assessments and provide patient education
Maintain timely, accurate,
and compliant clinical documentation
Complete a minimum of 2,946
outpatient visits annually
Participate in quality
assurance activities, clinical meetings, and performance audits
Support quality improvement
initiatives and clinical protocols
Participate in community
outreach, education, and student precepting as assigned

Requirements:

Master’s Degree in Nursing
from an accredited Nurse Practitioner program
Active Indiana RN license
and Nurse Practitioner certification
Prescriptive authority with
active DEA registration
Current CPR certification
1–3 years of Nurse
Practitioner experience or significant RN experience in ER or ICU
