The Clinical Team Manager RN oversees coordination of home health and hospice services, ensuring compliance with standards of practice and company procedures. Responsibilities include review and approval of patient information, orders, and documentation; oversight of patient care and staff management; education and training of clinicians; and facilitation of relationships among physicians, referral sources, patients, caregivers, and employees.
- Review on‑call coordination notes reports daily.
- Communicate and introduce services to patients and families, coordinating with the sales team as needed.
- Provide educational material on medical diagnoses, care provision, psychosocial aspects, and end‑of‑life care.
- Assist with maintaining provider requirements and resolve issues with providers, sales, and clinical staff.
- Process workflow, coordination notes, and administrative tasks in a timely manner.
- Back up the intake coordinator with referrals, ensuring clear identification of source.
- Attach referral paperwork to medical records promptly.
- Communicate acceptance of referrals clearly with referral sources.
- Back up the Patient Service Coordinator with rescheduling missed and declined visits.
- Review patient schedules and approve schedule changes to ensure appropriate clinical skill mix.
- Follow up on orders when records are pending or unsigned.
- Stay up to date on internal announcements and ensure compliance with TCT policies and procedures.
- Provide orientation and in‑service training to staff on documentation standards.
- Assist the Branch Director and administrator during surveys.
- Attend staff meetings, in‑services, and community education functions.
- Address action items and support team goals.
- Participate in administrative on‑call, supporting nurse and software management during off hours.
- Conduct continuous quality assessment and performance improvement activities.
- Complete onsite supervisory visits as assigned.
- Assume leadership when the branch director is absent, performing supervisory tasks and making hiring/termination recommendations.
- Manage referral intake and ensure timely assessment visits.
- Assist with patient review meetings and address care decisions.
- Review and approve coordination notes submitted by case managers.
- Review and approve all patient information submitted by licensed professionals.
- Review orders, approve or decline as appropriate; follow up with licensed professionals on corrections.
- Ensure all orders are complete and correct before routing for physician signature.
- Verify orders for requested medical supplies.
- Enter non‑admit information into records when no visit is made.
- Process wound score deviations and vital sign alert reports.
- Receive lab reports, assess normality, fax to physician, and upload confirmation.
- Initiate employee and patient infection reports as needed.
- Review evaluation documents and plans of care for accuracy.
- Process POC and verify correct start of care dates.
- Follow up on comprehensive assessments with documentation deficiencies.
- Perform ongoing chart audits per SOP.
- Assist with hospice item set data and error resolution.
- May perform licensed professional duties and participate in on‑call rotation as needed.
- Perform any additional duties assigned by supervisor.
- Previous experience in Home Care Home Base (HCHB) preferred; advanced computer skills preferred.
- Organized, detail‑oriented, and able to manage multiple projects simultaneously.
- Independent worker with strong oral and written communication skills.
- Self‑starter capable of working both independently and as part of a team.
