Registered Nurse RN Home Health

Elara Caring seeks a Registered Nurse (RN) to provide skilled nursing care in patients' homes, supporting more than 60,000 patients each day.

• Provide skilled nursing care to patients in their homes.
• Assess patient health status and identify physical, psychosocial, and environmental needs.
• Develop, implement, and manage individualized plans of care.
• Conduct initial evaluations and ongoing patient assessments.
• Coordinate care with physicians and interdisciplinary teams.
• Monitor patient progress and communicate changes in condition.
• Maintain accurate and timely clinical documentation.

• Current Registered Nurse (RN) license in the state of practice.
• 1+ year of clinical nursing experience.
• Ability and willingness to travel within the branch coverage area.
• Reliable transportation, valid driver’s license, and auto insurance.
• Ability to lift, push, pull, or assist patients up to 50 pounds.
• Associate or Bachelor’s Degree in Nursing preferred.
• Home Health or Hospice experience preferred.
• Experience with Home Care Home Base preferred.
