Registered Nurse RN Home Health
Elara Caring seeks a Registered Nurse (RN) to provide skilled nursing care in patients' homes, supporting more than 60,000 patients each day.
Responsibilities- Provide skilled nursing care to patients in their homes.
- Assess patient health status and identify physical, psychosocial, and environmental needs.
- Develop, implement, and manage individualized plans of care.
- Conduct initial evaluations and ongoing patient assessments.
- Coordinate care with physicians and interdisciplinary teams.
- Monitor patient progress and communicate changes in condition.
- Maintain accurate and timely clinical documentation.
- Current Registered Nurse (RN) license in the state of practice.
- 1+ year of clinical nursing experience.
- Ability and willingness to travel within the branch coverage area.
- Reliable transportation, valid driver’s license, and auto insurance.
- Ability to lift, push, pull, or assist patients up to 50 pounds.
- Associate or Bachelor’s Degree in Nursing preferred.
- Home Health or Hospice experience preferred.
- Experience with Home Care Home Base preferred.
