JOB OVERVIEW
WEEKEND REGISTERED NURSE RN – HOME HEALTH $7,500 Sign‑On Bonus. Looking for a 3‑day work week with full‑time benefits? This Weekend RN opportunity offers a consistent schedule with flexibility built in.
SCHEDULE- 12‑hour shifts
- Saturday and Sunday required
- One weekday of your choice
- Full‑Time status
St.Joseph and Elkhart Counties. Must be willing to travel within coverage area.
RESPONSIBILITIES- Manage and coordinate your own home health caseload
- Complete Start of Care, follow‑up, and discharge visits
- Perform comprehensive patient assessments
- Develop and update individualized plans of care Complete OASIS documentation accurately and timely
- Educate patients and families on medications and disease management
- Monitor patient progress and report changes to physicians
- Collaborate with interdisciplinary care team members
- Ensure documentation meets compliance and regulatory standards
- Active, unrestricted RN license
- At least 1 year of nursing experience
- Willingness to travel within the coverage area
- Reliable vehicle, valid driver’s license, and auto insurance
- Home Health experience preferred
- Can sit, stand, bend, lift and move intermittently and be able to lift 50‑100lbs.
