Visit Nurse LPN Palliative Care - Non Exempt

Provide holistic, patient‐ and family‐centered palliative care in the patient's residence, performing comprehensive assessments and executing therapeutic regimes and medication plans as prescribed. The role includes triaging symptom management via telephone, coordinating care with physicians or nurse practitioners, and facilitating referrals to community resources. This position requires approximately two nursing visits per day and is scheduled for Monday, Wednesday, and Friday from 8:00 am to 5:00 pm.

• Assess patient needs and evaluate symptoms and progress through comprehensive assessment.
• Execute therapeutic regimes, medications, and plans of care as prescribed.
• Provides holistic, patient/family-centered care in the patient’s residence to improve quality of life.
• Consult and collaborate with the palliative care interdisciplinary team and other agencies involved in patient care.
• Initiate or facilitate referrals to appropriate agencies or community resources.
• Assesses all aspects of the patient’s pain and comfort, developing and implementing an individualized pain management plan.
• Triages symptom management via telephone, coordinating with the patient’s physician or nurse practitioner.
• Communicates identified patient needs to appropriate disciplines via verbal and written reports of the current status of the patient.
• Collaborates with caregivers, physicians, and pharmacists to manage symptoms and educate caregivers on new medications and/or treatments.
• Collaborates with the palliative care physician or nurse practitioner regarding the plan of care and new orders needed at the time of admission.
• Reports appropriate information to the physician/nurse practitioner and the interdisciplinary team, ensuring reports are clear, concise, and timely.
• Completes patient care documentation in EMR in a timely manner, including medications, assessments, interventions, and outcomes.
• Ensures the ECF chart’s Agency section contains necessary information for Agency/ECF compliance, if applicable.
• Documents all nurse practitioner and physician orders, IDG notes, and patient notes on the day work is performed.
• Assists with continuity of care between team members and other service providers through clear verbal and written reports.
• Teaches patient self‑care, medication indications, side effects, and contraindications.
• Assists the patient’s family (and ECF staff if applicable) with understanding medical illnesses, plan of care, and patient needs.
• Serves as preceptor to other staff nurses, aides, and students.
• Complies with agency policies to prevent fraud and abuse.
• Demonstrates knowledge of palliative care and hospice philosophy.
• Arrives to work and meetings on time and ready to perform duties.
• Respects confidentiality of patients, families, and agency employees.
• Attends nursing meetings, educational conferences, and other professional development activities as required.
• Participates in nursing validations and agency infection control plan.
• Adapts to changes, maintaining flexibility while meeting organizational commitments under time pressure without compromising quality of care.
• Performs other duties that may be assigned.
• Participates in nursing call rotations with other hospice nursing staff.

• Current nursing licensure in Indiana and CPR certification.
• Prior experience in palliative care/hospice nursing is required.
• Ability to perform tasks requiring standing, walking, sitting, climbing stairs, stooping, kneeling, twisting, bending, reaching overhead and in front of the body, and using both hands, wrists, and fingers simultaneously.
• Ability to speak conversationally in English/American language, hear conversations and bodily functions, palpate the human body, and observe patients with adequate vision acuity and depth perception.
• Ability to work in varied environmental conditions, including adverse weather, dust, fumes, tobacco smoke, noise, confined spaces, and areas with poor or improper lighting and glare.
• May be required to work in steady or rotating shifts.
• Must own and operate a personal vehicle with a valid driver’s license and current automobile insurance.
