* We are seeking a compassionate and detail-oriented
** Medicaid Eligibility Specialist
** to support older adults and their caregivers through the Medicaid application and eligibility process. In this role, you will serve as a trusted resource, helping participants navigate Medicaid requirements while ensuring timely and accurate submission of applications, re-determinations, and supporting documentation.

** Key Responsibilities*
* + Guide participants and families through Medicaid applications, approvals, and re-determinations.
+ Conduct home visits to complete applications and gather required information.
+ Collaborate with county agencies, participants, caregivers, and internal teams to ensure eligibility requirements are met.
+ Educate families on Medicaid rules and regulations, including Long-Term Care Medicaid, trusts, and spousal impoverishment provisions.
+ Monitor and track eligibility status, maintaining accurate records and documentation.
+ Complete HMO disenrollment paperwork and communicate progress to enrollment and leadership teams.
+ Maintain strict confidentiality and compliance with all federal, state, and organizational requirements.
** Qualifications*
* + High school diploma or equivalent required.
+ Comprehensive knowledge of Medicaid programs, eligibility requirements, and benefits.
+ Understanding of intake practices and geriatric assessment principles.
+ Knowledge of community resources available to seniors and their families.
+ Minimum of one (1) year of experience working with frail, elderly, or aging populations.
+ Strong communication, organizational, and customer service skills.
+ Ability to work independently while building collaborative relationships with participants, families, and team members.
