About the Position
The RN Case Manager coordinates patient care across healthcare settings, ensuring optimal outcomes, cost-effective care, and smooth transitions. The nurse collaborates with physicians, patients, families, and multidisciplinary teams to develop, implement, and evaluate individualized care plans.
Requirements
Requirements
Active RN License in [State] or compact license
Minimum 1–2 years nursing experience , preferably in acute care or case management
BLS required ; additional certifications (ACLS, CCM) preferred
