Home Health LVN Position Summary

Work as a vital member of the care team, providing hands‑on support to patients while utilizing AI‑supported technology to simplify your documentation process. Spend more time delivering care and less time on paperwork.

• Assist with medication management, wound care, and health monitoring.
• Use AI systems to assist in completing clear and accurate notes during visits.
• Focus on patient interaction and quality of care by reducing administrative burden.

• Graduate of an accredited Licensed Practical/Vocational Nurse program.
• 1 year experience in a hospital, nursing home, home health, or experience working with terminally ill patients.
• Work Experience:
  
  Minimum 1 year as an LVN (6 months home care preferred).
• Certifications/Licenses:
  California Licensed Vocational Nurse required upon hire;
  Infusion and Venipuncture Certification required;
  National Provider BLS – American Heart Association within 30 days of hire. Valid driver’s license and auto insurance required upon request.

• Active and unencumbered Licensed Vocational/Practical nurse license in state(s) of employment required.
• Preferred within 30 days of hire:
  National Provider BLS - American Heart Association.

The demands of this role necessitate a team member to effectively perform essential functions. Adaptations can be made to accommodate team members with disabilities. Regular standing, walking, and manual dexterity are fundamental, along with the ability to lift and move objects up to 50 pounds. Visual acuity requirements include close and distance vision, color and peripheral vision, depth perception, and the ability to adjust focus.

In a healthcare setting, exposure to bodily fluids, infectious diseases, and conditions typical to the field is expected. Routine use of standard medical equipment and tools associated with clinical care is essential.

Pay Rate: $33.05 – $40.82
