We are recruiting a Social Services Director for a newly built, high-end skilled nursing facility in Mission Viejo, CA, backed by a respected California CCRC operator.

This is an opportunity for an experienced SNF social services leader who can independently own the department, maintain a consistent onsite presence, and serve as a steady resource for residents, families, the interdisciplinary team, and admissions partners.

Position Summary

The Social Services Director leads the social services function for a skilled nursing facility in Mission Viejo. This role requires a hands-on practitioner who can manage the full scope of SNF social services responsibilities, partner closely with nursing, rehabilitation, admissions, and facility leadership, and maintain strong follow-through through admissions, care planning, discharge, and resident or family escalations.

The successful candidate will bring the judgment and presence to run the department independently, particularly in a setting without confirmed dedicated social services support. Full-time onsite availability and dependable attendance are essential.

Key Responsibilities
  • Lead the skilled nursing facility's social services department and oversee day-to-day psychosocial support for residents and families.
  • Complete and maintain psychosocial assessments, care plans, discharge planning documentation, and required social services records.
  • Participate actively in interdisciplinary care plan meetings and collaborate with nursing, rehabilitation, dietary, activities, and other facility departments.
  • Partner with admissions to support smooth resident transitions, identify psychosocial needs early, and coordinate appropriate resources.
  • Guide residents and families through care transitions, discharge options, resident rights concerns, financial or facility resource needs, and complex family dynamics.
  • Coordinate referrals to outside agencies and facility resources as appropriate.
  • Support compliance with CMS requirements, California skilled nursing regulations, resident rights standards, and facility policies.
  • Address resident, family, and interdisciplinary concerns promptly, escalating issues appropriately and maintaining clear communication with the Administrator.
  • Maintain an organized, responsive social services operation that supports census flow, discharge readiness, and a positive resident and family experience.
Qualifications
  • Proven Social Services Director or comparable social services leadership experience in a skilled nursing setting.
  • Demonstrated ability to independently manage SNF social services responsibilities without relying on dedicated direct support.
  • Strong working knowledge of psychosocial assessments, care planning, discharge planning, resident rights, and skilled nursing compliance expectations.
  • Experience partnering with admissions and interdisciplinary teams on resident transitions, family communication, and discharge barriers.
  • Sound judgment, strong documentation discipline, and the ability to remain organized when priorities shift.
  • A consistent, reliable full-time onsite work history and the ability to maintain a dependable schedule in Mission Viejo, CA.
  • Familiarity with the Mission Viejo or broader Orange County skilled nursing market is a plus.
