Position: Nurse Care Coordinator (RN)

Nurse Care Coordinator (RN)

The Nurse Care Coordinator (RN) plays a vital role in advancing high‑quality, patient‑centered care within our Federally Qualified Health Center (FQHC). They will utilize care coordination, case management and resource management to improve the patient’s health, reduce healthcare costs, and enhance the overall patient experience. The Coordinator collaborates with patients, primary care physicians, and other team members—including outside entities and community health partners—to help patients achieve their healthcare goals.

The work may encompass medical, mental health, substance use, and social determinants of health.

Key Responsibilities
  • Support team‑based care delivery using FQHC and Patient‑Centered Medical Home (PCMH) principles.
  • Maintain a caseload of patients while performing disease‑management support and health coaching.
  • Work Monday‑Friday only—no evenings, weekends or holidays.

Requirements
  • Current Vermont RN license in good standing.
  • Associate or Bachelor’s degree in Nursing (BSN preferred).
  • Minimum of 2 years of RN experience preferred.
  • Experience in primary care, care coordination, case management, population health, or community health strongly preferred.
  • Knowledge of social determinants of health and community‑based resources.
  • Strong communication, organization, and patient advocacy skills.
  • Commitment to the mission and values of community health and serving underserved populations.
