Overview
The Care Coordinator is to promote and coordinate team-based care of patients through effective communication with patients, their caregivers/families, community resources, and their provider. Promotes high-quality care that is patient and family centered. The wound care nurse specializes in assessing and treating patients with complex wounds, ostomies, or continence conditions. Works closely with members of patient care teams to ensure that personalized treatment plans are followed correctly to prevent future infection or injury.
Common types of complex patient wounds include ulcers, burns, pressure injuries, diabetic foot wounds and skin tears.
- Coordinate team-based care through effective communication with patients, caregivers/families, community resources, and providers.
- Promote high-quality, patient- and family-centered care.
- Assess and treat patients with complex wounds, ostomies, or continence conditions as part of wound care.
- Collaborate with members of patient care teams to ensure personalized treatment plans are followed correctly to prevent future infection or injury.
- Address common complex wound types, including ulcers, burns, pressure injuries, diabetic foot wounds, and skin tears.
- Current license with MS State Board of Nursing
- Current BLS
- Current ACLS
