Job Purpose:
- The Nurse Practitioner - Primary Care at SMART CHOICE MEDICAL SERVICES LLC will provide comprehensive healthcare services to patients, focusing on health promotion, disease prevention, and the management of acute and chronic conditions. The role involves collaborating with physicians and other healthcare professionals to ensure high-quality patient care and optimal health outcomes.
Key Responsibilities:
Conduct comprehensive patient assessments, including health history, physical examination, and diagnostic testing.
Develop, implement, and evaluate individualized treatment plans for patients, ensuring they align with best practice guidelines.
Prescribe medications and other treatments as necessary, in accordance with state and federal regulations.
Educate patients and their families on health maintenance, disease prevention, and management of chronic conditions.
Collaborate with physicians and other healthcare team members to coordinate patient care and improve health outcomes.
Maintain accurate and up-to-date patient records, ensuring confidentiality and compliance with healthcare regulations.
Participate in continuous professional development and stay current with advancements in primary care practice.
Contribute to quality improvement initiatives within the practice to enhance patient care and service delivery.
Required
Education:
Master’s degree in Nursing from an accredited institution.
Current and valid Nurse Practitioner license in the state of practice.
Certification as a Nurse Practitioner in Primary Care.
Required Experience:
Minimum of two years of clinical experience as a Nurse Practitioner in a primary care setting.
Proven track record of providing high-quality patient care and managing diverse patient populations.
Experience in conducting physical examinations, diagnosing conditions, and developing treatment plans.

Skills and Abilities:
Strong clinical skills with the ability to assess, diagnose, and treat a broad range of health issues.
Excellent communication and interpersonal skills for effective patient interaction and collaboration with healthcare teams.
Proficiency in electronic health records (EHR) systems and other healthcare technologies.
Ability to work independently and make informed decisions in a fast-paced environment.
Strong organizational skills and attention to detail for maintaining accurate patient records and documentation.
Commitment to ongoing professional development and staying current with medical advancements and best practices.
Physical Requirements:
Ability to stand, walk, and move for extended periods during patient care activities.
Frequent bending, reaching, and lifting of supplies or equipment up to 25 pounds.
Manual dexterity to perform patient care procedures and operate medical instruments.
Visual and auditory acuity sufficient to assess and monitor patient conditions, and communicate effectively.
Physical stamina to respond quickly in emergency situations and manage high-demand periods.
