Licensed Practical Nurse (LPN)

Middleton Oaks Health & Rehabilitation is seeking dedicated Licensed Practical Nurses (LPNs) to join our team. This role provides both direct patient care and clinical support within a skilled nursing and rehabilitation setting under the supervision of a Registered Nurse (RN). LPNs can earn up to $28.00 per hour, including applicable shift differentials.

• 7:00 AM – 3:00 PM
• 3:00 PM – 11:00 PM
• 11:00 PM – 7:00 AM

• Provide direct patient care, including assistance with activities of daily living (ADLs) such as bathing, dressing, eating, and mobility
• Administer medications and perform basic wound care
• Monitor resident health status and record vital signs
• Communicate changes in condition to the interdisciplinary care team
• Maintain accurate and timely clinical documentation
• Supervise and support Certified Nursing Assistants (CNAs)
