Licensed Practical Nurse

This is licensed practical nursing work involving the provision of direct nursing care, medication administration, treatment support, health monitoring, documentation, and communication with individuals served, families, and healthcare staff in an institutional or residential care setting.

The incumbent performs nursing duties under applicable licensure requirements, agency policies, and supervisory direction. Work requires clinical judgment within the scope of practical nursing practice, attention to detail, documentation accuracy, communication skills, and the ability to respond appropriately in an often-changing care environment.

Examples of work include assessing and evaluating illnesses or injuries; documenting nursing observations, care, and treatment; administering prescribed medications and treatments; carrying out physician orders; communicating with individuals served, families, supervisors, and healthcare staff; accompanying individuals served to medical appointments; performing urine drug screens; using computers and applicable software to complete required documentation; monitoring health-related needs; reporting changes in condition; following Board of Nursing rules and regulations;

following Intermediate Care Facility for Individuals with Intellectual Disabilities program policies; and performing related duties as assigned.

Typically requires a certificate from an approved school of nursing, licensure as a Licensed Practical Nurse, and 0-4 years of experience.

Essential functions include providing licensed practical nursing care; assessing and documenting illnesses, injuries, care, and treatment; administering medications and treatments as prescribed; carrying out physician orders; communicating effectively with individuals served, families, healthcare staff, supervisors, and other agency personnel; accompanying individuals served to medical appointments; performing urine drug screens; using computers and applicable software for documentation; maintaining accurate health-related records; following applicable Board of Nursing rules and regulations, agency policies, and program requirements;

maintaining professionalism and confidentiality; responding appropriately to an unpredictable work environment; and performing related nursing duties as assigned.
