Responsibilities
- Assess & plan complete in-home RN assessments; craft individualized care plans with client/family approval.
- Monitor & document review daily care logs, track condition changes, and interventions in our HIPAA-secure EMR; elevate concerns promptly.
- Collaborate & advocate, coordinate with physicians, therapists, and schedulers to keep care seamless and client-centered.
- Active Registered Nurse (RN) license in Georgia
- Bachelor’s in Nursing or related human‑services field – OR – 2+ years’ geriatric/home‑care experience
- Solid clinical assessment skills and a passion for problem‑solving
- CPR/First Aid certification and a valid driver’s license
- Warm, professional communication that builds trust with families and field staff
Demonstrates expertise in conducting comprehensive in‑home RN assessments and developing individualized care plans, while effectively collaborating with healthcare professionals to ensure seamless, client‑centered care. Strong clinical assessment skills and problem‑solving abilities are essential for monitoring patient conditions and advocating for their needs.
