LPN - Long Term Acute Care Facility

Assignment Duration:
This is a 13-week contract position.

Schedule:

12-hour shifts, , with 84 hours biweekly (48 hours one week and 36 hours the next) including weekends and holidays as needed.

Job Description:

We are seeking a dedicated Licensed Practical Nurse (LPN) for a Long Term Acute Care Facility. The role involves delivering quality nursing care to a diverse patient population, including geriatrics and patients requiring rehabilitation, ventilatory support, and post-acute care.

Responsibilities:

• Provide nursing care to patients in compliance with facility standards and state regulations.
• Administer medications and treatments as prescribed by physicians.
• Monitor patient vital signs and report changes in condition to the nursing team.
• Assist patients with activities of daily living (ADLs) and ensure a safe environment.
• Collaborate with interdisciplinary teams to develop and implement patient care plans.

Required Experience / Certifications / Licensure:

• Current LPN license with IV certification.
• Minimum of two (2) years of experience in a Long Term Acute Care (LTAC) setting.
• Experience with ventilated and tracheostomy patients is required.
• BLS certification is required; ACLS certification is preferred.
• Experience in larger facilities (400+ beds or level 2 trauma) is preferred.
