Wiggins Community Homes, 509 Stapp St., Wiggins, MS 39577 PAY: $ 36,624.00 SCHEDULE/
SHIFTS:
Monday – Friday 8:00 a.m. to 4:30 p.m. with variations as necessary. RESPONSIBILITIES:
Serves as the primary liaison between individuals served through the community program and their families/correspondents. Provides support to individuals through socialization group training. Coordinates various appointments including monthly visits to community vendors and families. Explains general medical and psychiatric recommendations to families/surrogates and obtains consent for surgery or other treatment. Counsels individuals served and their families on adjusting to routines and life at the facility.
Prepares and coordinates discharge plans. Participates in interdisciplinary team meetings to plan for the training/habilitation and/or discharge of individuals served. Coordinates services among agencies contributing to individual's welfare. Prepares social histories by interviewing individuals and families and making other necessary contact. Supervises and works on floor with direct care staff. Related or other duties as required or assigned.
Typically requires a Bachelor's Degree and 4-7 years of experience. Agency preferred qualifications:
Requires a Bachelor's Degree in psychology or related mental health field and 4-7 years of experience. Must be eligible to drive a State of Mississippi vehicle. Must have at least 1 year of experience working with people with IDD. Must be eligible to drive a State of Mississippi vehicle.
