This position is accountable and responsible for the provision and coordination of clinically competent care including assessment, planning, intervention and evaluation for an assigned group of patients. As a member of the End Stage Kidney Disease (ESKD) health care team, this position participates in decision‑making, teaching, leadership functions, and quality improvement activities that enhance patient care outcomes and facility operations.
Principal Duties and Responsibilities- Perform all essential functions within Client policy, procedures, standards of nursing practice, state and federal regulations.
- Work under the direction of the Supervisor and guidance of Clinical Educator, Preceptor, or other RNs.
- Collect and analyze patient data pre‑during‑post hemodialysis; document and adjust treatment plans as needed.
- Assess, collaborate, and document patient/family learning needs; provide education.
- Lead safe and effective delivery of hemodialysis therapy to assigned patients in collaboration with staff.
- Administer medications per protocol; document justification and effectiveness.
- Initiate or assist with emergency response measures.
- Serve as resource; participate in staff training and orientation.
- Ensure correct laboratory procedures; reschedule missed/insufficient collections.
- Update nursing assessment and plan of care; collaborate with Interdisciplinary Team.
- Ensure patient awareness of transplant and treatment modality options.
- Perform other duties as assigned by Supervisor.
- Direct patient care with heavy lifting, moving patients, and assisting ambulation; may use equipment aids.
- Frequent, prolonged standing; ability to bend over.
- May need to move machines and equipment up to 200 lbs; water/chemical solutions up to 30 lbs up to 5 ft.
- Work environment: health care facility with air temperature control, moderate noise.
- Exposure to infectious and contagious diseases/materials.
- Desk and computer work; interaction with patients, staff, physicians.
- May require travel to training sites or other facilities; may perform essential functions elsewhere with same demands.
- Graduate of accredited School of Nursing.
- Current appropriate state licensure.
- Current or completed CPR BLS Certification.
- Must meet state practice requirements.
- Minimum of 9 months RN experience.
- Minimum 3 months chronic/hemodialysis RN experience; 12 months recent dialysis experience required.
- 1 year Nephrology Nursing experience in last two years as RN.
- Floating requirement: must float to area sites within 50 miles of main site.
- Color blindness: must take Ishihara test; accommodations may be considered if fail.
- Must be able to commit to 40 hrs + overtime and floating; minimum guarantee 36 hrs/week.
Shifts will be discussed during interview. Shift: 8 hours, days. Commitment: 40 hours/week plus overtime and floating. Minimum guarantee: 36 hrs per week.
