Schedule: Typical schedule is Monday–Friday, evening/weekend on‑call rotation as required.

Job Summary

The RN – Case Manager provides skilled nursing, care, and case management for assigned patients in their place of residence; admits patients onto services; coordinates and integrates services to ensure continuity of care; facilitates the achievement of quality, clinical and cost outcomes; negotiates, procures and coordinates services and resources needed by the patient or family; creates opportunities and systems to enhance outcomes;

and uses the nursing process of assessment, planning and implementation of care, including patient/family instruction and evaluation through an inter‑disciplinary team approach.

Essential Duties and Responsibilities
  • Provides skilled nursing and coordinates care to patients and family members in a residential setting through a team approach and based upon systematic evaluation as authorized by a physician.
  • Observes, assesses, evaluates, and plans interventions in relation to patients’ physical and psychosocial statuses, including changes in condition and/or reactions to treatments or medications for the duration of contact with patients.
  • Oversees care provided by Home Health Aides and CNAs to assigned patients and evaluates compliance with plan of care.
  • Assures continuity of care through effective communication, information exchange and case conferences with other disciplines, medical providers, community health practitioners, support and social agencies.
  • Provides quality care and appropriate symptom management for positive clinical outcomes and promotes optimal health and quality of life through teaching, counseling, appropriate interventions and referrals.
  • Maintains accurate, current and complete patient records in accordance with Agency standards and OASIS/HIS/HOPE requirements as well as knowledge of current reimbursement rules and regulations.
  • Participates in team scheduling coordination and “on‑call” rotation schedule as necessary to meet patient care needs by assessing and evaluating patient needs via telephone and advising or making home visits as required.
  • Participates in quality improvement activities, acquires knowledge and accepts responsibility of quality metrics related to care delivery and attends in‑service training as assigned.
  • Manages and meets productivity and caseload expectations in line with the agency’s standards.
  • Performs other duties as assigned by supervisor.
Requirements

Minimum Qualifications

  • Current Registered Nurse (RN) license in the State of Montana (BSN preferred).
  • Valid driver’s license, auto insurance, and access to reliable transportation.
  • Preferred:
    Two years of recent clinical experience in acute care, home health, or hospice, including skills such as venipuncture, catheter care, wound management, and parenteral therapy.
