Care Manager LPN LVN - Behavioral HEALTH

Responsibilities

• Conduct social and family history, psychiatric history, mental status exam, and other relevant assessments for accurate evaluation of patients' psychiatric and mental problems.
• Collect and document medical and behavioral information specifically related to patients' outpatient services.
• Provide care to both adult and adolescent populations.

• Montana Licensed Practical Nurse (LPN).
• Graduate of an accredited school of nursing.

• Associate's Degree in Nursing.
• 3 years experience working with adult and/or adolescent populations and familiarity with a broad range of psychiatric disorders.

Base pay range: $29.78 - $46.25 per hour. Additional compensation may be available for shift differentials, standby/on-call, overtime, premiums, extra shift incentives, or bonus opportunities.
