Position: Hospice RN, Home Health RN-2

Hospice Registered Nurse (RN)

Job Summary:

The Hospice RN provides compassionate, end-of-life care to patients with terminal illnesses. The role focuses on pain management, emotional support, and improving the quality of life for patients and their families during the final stages of life.

Key Responsibilities
  • Assess patient condition and develop individualized hospice care plans
  • Provide pain and symptom management
  • Administer medications and treatments as prescribed
  • Offer emotional support to patients and families
  • Educate families on end-of-life care and coping strategies
  • Coordinate care with physicians, social workers, and care teams
  • Maintain accurate documentation and compliance
  • Respond to patient needs and emergencies
Required Qualifications
  • Associate’s or Bachelor’s Degree in Nursing (ADN/BSN)
  • Active RN license
  • BLS/CPR Certification
Experience Required

1–3+ years of nursing experience (Hospice or Palliative care preferred).

Home Health Registered Nurse (RN)

Job Summary:

The Home Health RN provides skilled nursing care to patients in their homes. This role focuses on patient recovery, chronic disease management, and promoting independence through personalized care plans.

Key Responsibilities
  • Conduct patient assessments and develop care plans
  • Provide wound care, medication administration, and IV therapy
  • Monitor patient progress and adjust care as needed
  • Educate patients and families on disease management and self-care
  • Coordinate with physicians and healthcare teams
  • Maintain accurate patient records and documentation
  • Ensure compliance with healthcare standards
Required Qualifications
  • Associate’s or Bachelor’s Degree in Nursing (ADN/BSN)
