Home Health / Hospice
LocationMissoula, MT and surrounding areas
StatusFull-Time (Rotating Schedule)
ScheduleRotating 7 days on / 7 days off. Coverage includes evenings, nights, weekends, and holidays occurring during scheduled weeks. Available by phone at all times during the assigned on-call week. Required to make home visits as needed based on triage assessment.
Position SummaryThe On-Call Nurse role ensures timely triage, intervention, and coordination of care during non-business hours. The nurse functions autonomously, prioritizing urgent issues, making home visits when necessary, and supporting continuity of care for patients in the community. The schedule follows a 7-days-on / 7-days-off rotation.
Patient Care & Triage- Provide telephone triage for urgent clinical needs, symptom changes, caregiver concerns, and equipment issues.
- Assess patient status via phone and/or in-person visits using clinical judgment and agency protocols.
- Perform home visits for urgent issues such as pain crisis, respiratory distress, falls, wound complications, or other acute changes.
- Deliver skilled nursing interventions, including medication administration, wound care, assessments, and stabilization.
- Ensure patient safety and support caregivers with teaching, reassurance, and guidance.
- Communicate significant changes to the primary nurse, interdisciplinary team, and physician as needed.
- Coordinate with the on-call provider, hospice physician, or NP for urgent orders or guidance.
- Document all triage calls, assessments, interventions, and visits in the EMR in a timely manner.
- Arrange after-hours equipment needs (e.g., oxygen, supplies) through approved vendors or internal DME when applicable.
- Adhere to agency policies, ACHC standards, and CMS Home Health/Hospice Centers of Practice.
- Participate in performance improvement activities.
- Maintain accurate and complete clinical documentation.
- Escalate concerns about patient safety, abuse/neglect, or environmental risks promptly.
- Education & Licensure (Required):
Current RN license (state-specific) in good standing. - CPR certification.
- Valid driver’s license, reliable transportation, and proof of auto insurance.
- Minimum 1–2 years of nursing experience in Home Health, Hospice, ER, ICU, Med/Surg, or similar setting.
- Strong clinical assessment and triage skills required.
- Experience with EMR documentation preferred.
