The RoleReporting directly to Greater Good Health’s Clinic Manager, the Licensed Practical Nurse (LPN) will lead patient care for GGH’s primary care clinic in Missoula, MT. The role will deliver an exceptional patient experience by working closely with the Nurse Practitioner to provide holistic, high‑touch, coordinated care for a panel of senior patients.
Day‑to‑Day ResponsibilitiesDeliver exceptional clinical care to patients and support functions related to patient care activities.
- Actively engage and manage patient panels in partnership with the Nurse Practitioner, acting as an extension of the NP to support delivery of high‑touch, high‑quality care.
- Act as patients’ health care advocate, helping patients navigate necessary health care resources to support their care plan.
- Ensure efficient clinic flow by rooming patients in a timely manner and assisting the provider with staying on schedule.
- Complete pre‑visit activities including gathering and verifying all relevant information about patients’ health, preparing charts for the provider, prepping exam rooms, and escorting patients from the lounge to the room.
- Conduct visit activities including taking vitals, performing appropriate screenings, recording patient histories, reviewing medications, and preparing patients for any procedures or tests in the exam room.
- Provide patient education on preventative measures, lifestyle management, and disease management.
- Under the direction of the Provider, offer MA/LNP appropriate visits, such as administering vaccinations and performing procedures (e.g., ABI, spirometry, EKGs, etc.).
- Oversee the laboratory and related activities, including reviewing lab orders, drawing blood, collecting samples, and preparing specimens for laboratory shipment or analysis.
- Support the check‑out process for patients.
- Support post‑visit activities; facilitate referral completion (specialty, lab, imaging, DME, etc.), patient care coordination activities, request external medical records, and conduct follow‑up outreach to patients and/or caregivers, as appropriate.
- Assist patients with scheduling for follow‑up visits, diagnostic imaging and/or labs to ensure completion.
- Handle pre‑authorization requests.
- Process medical refill requests, track status, and schedule patients as required to fulfill refill requests.
- Ensure efficient patient flow by serving as the air traffic controller and flexing when needed to support patient rooming.
- Handle back‑office phone calls.
- Maintain equipment for exam rooms and laboratory; restock exam rooms while maintaining cleanliness and organization and dispose of contaminated items according to protocol.
- Disinfect and clean treatment rooms following patient examinations; maintain a safe, secure, and healthy work environment by establishing and following standards and procedures.
- Maintain all logs and required checks (e.g., refrigerator temperatures, emergency medications, expired medications, oxygen, etc.) per protocols.
- Manage immunization program from ordering, storing, and handling all vaccines; ensure the vaccine management plan is adhered to.
- Manage medication inventory.
- Monitor patient care quality and clinical outcome KPIs.
- Promote high‑quality patient care and outstanding patient experience as reflected in patient satisfaction scores.
- Collaborate with all members of the team to support one another and strive to ensure all peers feel successful in their day.
- Welcome all and care empathetically for patients and team members.
