Summary Of Position
The Licensed Practical Nurse (LPN) is integral to our team’s success, ensuring high‑quality holistic nursing care in accordance with nursing practice standards and healthcare regulations. The LPN leads or directs licensed and non‑professional staff in delivering direct resident care and support functions.
Essential Job Functions Nursing Care Functions- Conduct daily nursing functions in compliance with current rules, regulations, and guidelines governing long‑term care facilities and assisted living communities.
- Ensure that all nursing personnel assigned to the LPN comply with written policies and procedures, including the safety program.
- Admit, transfer, and discharge residents with appropriate documentation.
- Orient new residents and family members to the facility.
- Make rounds with healthcare providers, hospice staff, and home health staff as needed.
- Utilize nursing personnel qualifications according to the resident’s care plan, service plans, and healthcare provider orders.
- Complete daily clinical and environmental rounds, evaluating resident care and reporting significant events.
- Meet and resolve issues with residents and/or family members; report significant issues to supervision.
- Participate in facility surveys (inspections) as requested by management.
- May be responsible for clinical programs such as dementia, wound care, quality assurance, infection control, admissions, and falls.
- Complete initial assessments and assist or develop interim care plans/service plans.
- Update, review, and transcribe care plans that identify goals, problems, approaches, and revisions based on nursing needs and provider orders.
- Assist with service plans as directed by the Director of Nursing.
- Communicate with healthcare providers to review treatment plans, complete orders, progress notes, etc., in accordance with established policies.
- Review the resident’s medical record for treatments, medication orders, diets, labs, etc., for implementation and follow‑up.
- Requisition and arrange diagnostic and therapeutic services as ordered by the provider, following established procedures.
- Communicate department policies and procedures to personnel, residents, and visitors.
- Communication via electronic or written (24‑hour report) and oral reports concerning changes in resident condition and shift activities to nursing management.
- Fill out and submit accident/incident reports involving residents to the Director of Nursing.
- Complete mandatory documentation mandated by regulations to provide care services.
- Provide resident and family information for care conferences or other meetings.
- Report discrepancies regarding physician orders, diet changes, charting errors, etc., to the supervisor.
- Ensure clinically accurate documentation according to policy.
- Review resident medical documentation and progress notes from the interdisciplinary team to verify adherence to care plans. Report concerns to supervisor.
- Inform nursing personnel of new admissions and resident care needs; ensure rooms are ready for new admissions and greet newly admitted residents.
- Notify the resident’s healthcare practitioner and family of changes in the resident’s condition.
- Assist in arranging transportation for residents and third‑party services.
- Prepare and administer medications ordered by the healthcare provider.
- Communicate with pharmacy to ensure resident medications are available for administration.
- Maintain adequate floor stock of medications, supplies, and equipment to meet nursing needs; report deficiencies to the supervisor.
- Review medication administration record for completeness, accuracy, and correct transcription of provider orders; report concerns to supervisor.
- Dispose of drugs and narcotics as required, following established procedures.
- Supervise nursing unit to ensure all personnel perform assignments in accordance with acceptable nursing standards.
- Work with the Director of Nursing to resolve staff performance issues, including those that may lead to termination.
