In collaboration with the Hope Foundation Partners In Home Care will be providing clinical operations at the new Hope Hospice Center.
Your role as a CNA is not just a job but a heartfelt opportunity to provide comfort and support to those nearing the end of life. Our CNAs provide personal healthcare and related services to patients in the Hope Hospice Center. This position is vital in creating a safe and clean environment, working cooperatively with patients and their families. Our CNAs are key members of each patient’s care team, committed to making a meaningful difference daily.
CoreResponsibilities
- Patient‑Centered Care:
Provide personal care in various settings the patient prefers, including homes, nursing homes, assisted living facilities, eldercare facilities or community partner’s inpatient care settings. - Observation and Reporting:
Act as the “eyes and ears” for the broader hospice team, observing the patient’s condition and reporting any changes or concerns. - Team
Collaboration:
Work alongside passionate healthcare professionals, providing a supportive and compassionate environment for patients and their families. - Personal Hygiene Assistance:
Help with the patient’s personal hygiene needs, including bathing, dressing, grooming, and oral/mouth care. - Light Housekeeping:
Perform light housekeeping tasks related to the patient, such as making the patient’s bed and tidying the patient’s immediate environment. - Adaptability:
Be flexible and adaptable, ready to adjust to changing patient needs and care requirements.
- Education & Certifications (Required):
Completion of a state‑approved Nurse Aide training/competency program;
Active CNA license in good standing on the state registry. - Experience (Preferred):
One year of CNA or hospice/home‑care experience preferred;
Reliable transportation with a valid driver’s license and auto insurance. - Knowledge, Skills & Abilities:
Reliable and dependable in attendance and follow‑through;
Strong communication skills with patients, families, and team members;
Adaptable and flexible in a fast‑paced care environment;
Positive, compassionate attitude toward patient care;
Willingness to learn and grow with feedback and training.
