Department: Home Dialysis Program Reports To: Clinical Manager / Medical Director
The Home Dialysis RN is responsible for providing comprehensive training, clinical oversight, and ongoing support to patients performing home hemodialysis (HHD) or peritoneal dialysis (PD). This role ensures safe, effective, and compliant delivery of home therapies in accordance with CMS Conditions for Coverage, facility policies, and physician orders. The nurse serves as the primary clinical resource for home patients, caregivers, and the interdisciplinary team.
Essential Duties & Responsibilities Patient Training & Education- Conducts individualized training for home hemodialysis or peritoneal dialysis patients and caregivers.
- Ensures patient competency in machine setup, treatment initiation, monitoring, troubleshooting, and infection control.
- Provides education on diet, fluid management, medications, and emergency procedures.
- Documents training progress and competency evaluations in accordance with CMS requirements.
- Performs comprehensive assessments during monthly home visits and clinic follow‑ups.
- Reviews treatment logs, vital signs, lab results, and patient‑reported issues.
- Identifies complications early and escalates to the physician as needed.
- Adjusts care plans based on patient condition, labs, and physician directives.
- Maintains complete, accurate, and timely documentation for all patient interactions.
- Ensures compliance with CMS Conditions for Coverage, infection control standards, and facility protocols.
- Participates in audits, QAPI activities, and interdisciplinary team meetings.
- Maintains evidence‑of‑process documentation for survey readiness.
- Participates in the home program on‑call rotation for urgent patient needs.
- Provides clinical triage, troubleshooting, and escalation to the on‑call physician when appropriate.
- Documents all after‑hours calls per policy.
- Collaborates with nephrologists, dietitians, social workers, and technicians to support patient outcomes.
- Coordinates supply orders, equipment needs, and vendor communication.
- Assists with transitions of care, including hospitalizations and modality changes.
- Ensures adherence to infection control protocols and patient safety standards.
- Monitors environmental and equipment safety during home visits.
- Participates in emergency preparedness training and patient education.
- Active RN license in the state of practice.
- Minimum 1 year of nursing experience (dialysis experience preferred).
- Strong assessment, teaching, and communication skills.
- Ability to work independently in patient homes and clinic settings.
- Valid driver’s license and reliable transportation.
- Previous home dialysis (HHD or PD) experience.
- Experience with Fresenius, NxStage, or Baxter home dialysis systems.
- Familiarity with CMS Conditions for Coverage for ESRD facilities.
- Monday–Friday with flexibility for patient needs.
- Participation in on‑call rotation required.
- Occasional weekends depending on patient training schedules.
- Ability to lift and move equipment up to 40 lbs.
- Ability to perform home visits, including climbing stairs and navigating varied home environments.
- Prolonged standing, walking, and patient interaction.
