RN - Hospice Center (Healthcare)

The Partners In Hope Foundation in collaboration with Partners In Home Care is hiring for a Hospice RN at its Hope Hospice Center that will provide skilled, compassionate care to patients in an inpatient hospice setting. This role ensures comfort, dignity, and quality of life for terminally ill patients, while also supervising and directing the non-unionized Certified Nursing Assistants (CNAs). The RN works closely with interdisciplinary teams to develop and implement care plans that reflect the patients needs and wishes.

Provide comprehensive nursing care to terminally ill patients, ensuring symptom management and comfort through medication administration and interventions as per physician orders. Assess and respond to patient needs, ensuring a dignified and comfortable end-of-life experience.

Supervise and guide non-unionized CNAs, ensuring they provide appropriate personal care and support per the patients care plan, facility standards, and their job description.

Appropriately delegate nursing support tasks, such as assisting with activities of daily living (ADLs), patient hygiene, and mobility assistance, while ensuring safe, compassionate care.

Offer real-time guidance and mentorship to CNAs, providing feedback on performance while maintaining a respectful work environment.

Monitor CNA documentation of patient care to ensure it is accurate and complete.

As a unionized RN, you will adhere to the terms of the collective bargaining agreement, respecting union rules, work hours, and break schedules while carrying out your duties.

Work collaboratively with the nurse union, following contract terms and ensuring fair, respectful treatment of nursing staff.

While supervising CNAs, maintain a professional and respectful relationship with all staff, acknowledging and adhering to union guidelines.

Participate in interdisciplinary team (IDT) rounds, contributing to patient care plans, symptom management, and family education. Collaborate with other healthcare professionals to ensure holistic patient care.

Accurately document nursing assessments, interventions, and communications with CNAs, patients, and families. Ensure all care provided complies with regulatory and quality standards.

Provide emotional support, education, and guidance to patients' families and caregivers, assisting them in navigating the emotional and physical challenges of end-of-life care.

Adhere to infection control procedures and safety protocols to ensure a safe, comfortable environment for both patients and staff.
