Licensed Vocational Nurse

Vital Caring seeks a Licensed Vocational Nurse to provide compassionate home health and hospice care. The LVN delivers direct nursing services in patients' homes, including assessments, wound care, medication administration, and monitoring of chronic conditions. This role collaborates with RNs, therapists, and physicians to follow individualized care plans, document in the EHR, and educate patients and families. Responsibilities

• Provide direct nursing care to home health and hospice patients under RN and physician direction
• Perform patient assessments, monitor vital signs, and report changes in condition promptly
• Administer medications and treatments, including wound care and chronic disease management
• Document all care accurately and timely in the electronic health record
• Educate patients and families on care plans, medications, and symptom management
• Collaborate with an interdisciplinary team to support safe, goal-directed care at home
• Maintain infection control, safety, and regulatory compliance standards during all visits
• Participate in case conferences and training to support clinical excellence and growth
