Part-Time Care Coordinator
Central Intake & Telephone Reassurance
About the RoleSometimes care starts with a phone call.
Circle of Care St. Louis is seeking two part-time Care Coordinators to serve as the central intake point for our organization. This role supports older adults and adults with disabilities by processing client intakes, entering information into our case management system, following up with clients, and providing telephone reassurance.
This position is ideal for individuals who are organized, dependable, calm under pressure, and motivated by meaningful work. All employees must align with the Circle of Care St. Louis Culture Policy and demonstrate Competency, Commitment, and Capacity in their work.
Work Schedule- Part-time, Monday–Friday
- 9:00 AM – 1:00 PM
- 1:00 PM – 5:00 PM
Central Intake
- Receive and process incoming client intakes
- Enter intake information accurately into the case management database
- Follow up with clients to complete or clarify intake information
- Explain services and next steps clearly and respectfully
- Maintain confidentiality and follow organizational policies
Telephone Reassurance
- Conduct scheduled reassurance calls to clients
- Provide emotional support and social connection
- Identify concerns and follow established escalation procedures
- Document all calls accurately and on time
Communication & Follow-Through
- Track assigned tasks and complete them within scheduled hours
- Communicate intake status or concerns with leadership as needed
- Seek guidance when unsure and follow official procedures

- Can self-regulate emotions and remain calm in challenging situations
- Use problem-solving skills while following established procedures
- Communicate clearly, professionally, and compassionately by phone
- Are reliable, punctual, and consistent
- Can work independently while staying connected to a team

Preferred (not required):
- Experience in social services, customer service, intake, or call‑center work
- Experience working with older adults or adults with disabilities
